Each set of survey forms are available in various formats. There is a cover page and form compiled into a single PDF. The cover page is also available in a Word document and listed separately from the body of the form which is available in an Excel document.
Pick the option that works for your computer and print out both the cover page and the body of the form. Provide as much information as you like on the form and mail it in to the seasonal compiler whose address is provided on the cover page. To ensure sufficient time to tabulate the results for publication, submissions must be made by the 15th of the month following the end of the season.
